X線透視ガイド下で手技を行う医療従事者用の衝立型放射線防護具を開発―医療従事者の放射線白内障発生リスク低減へ―

東北大学の研究グループは、X線透視ガイド下で手技を行う医療従事者向けに、Cアーム式装置にも対応可能な衝立型放射線防護具を開発した。対象は内視鏡的逆行性胆管膵管造影(ERCP)手技で、術者用と麻酔科医用の2種類を設計。患者テーブルへ簡便に装着でき、近年普及するCアーム型透視装置でも使用可能とした。人体模型による評価では、術者の眼位置で散乱線を約90%、麻酔科医位置で約86%低減でき、従来の含鉛防護眼鏡を上回る遮蔽性能を示した。X線透視下手技では散乱線による水晶体被ばくが問題で、近年は放射線白内障リスクが従来想定より高いことが明らかになっている。本成果は、医療従事者の眼被ばく低減と安全性向上に貢献し、放射線防護対策の実用化・普及が期待される。

ABSTRACT

Medical personnel performing endoscopic retrograde cholangiopancreatography (ERCP) are at a high risk of developing radiation-induced injuries, such as cataracts, due to scattered radiation, making the use of appropriate radiation protection equipment essential. We developed two types of upright shields attached to a C-arm system (under-couch tube position): one for the operator and the other for the anesthesiologist. The purpose of this study was to evaluate the protective effect of these upright shields. We conducted a phantom study in typical ERCP settings and compared the scattered radiation doses between the C-arm system equipped with the upright shields and an over-couch tube system equipped with an ERCP-specific lead curtain. It was found that the upright shield for the operator reduced scattered radiation exposure by 90%, and the upright shield for the anesthesiologist reduced scattered radiation exposure by 86%. The upright shields for a C-arm system could be just as effective as the ERCP-specific curtain for an over-couch tube system. The use of the novel upright shields may eliminate the need for protective glasses and is expected to not only protect the eyes but also safeguard the thyroid and brain.
